On two stellations of the rhombic triacontahedron, see http://georgehart.com/virtual-polyhedra/equal-face-equal-vertex.html. They are the two listed as K and 2B near the bottom. I suspect this occurs as a result of having self-intersecting faces, the behavior of which is technically undefined in VRML97. But worth tracking down and checking.
A lot of these have it http://georgehart.com/virtual-polyhedra/stellations-triakis-tetrahedron.html. I think the examples support my initial diagnosis.
