Expand the command history feature from issue #139 so that it shows not only commands entered at the prompt, but also command representations of actions triggered in other ways—for example, using buttons or keyboard shortcuts.

We expect the command history to ultimately become a view of a general undo history. The changes that address this issue should be designed with that future in mind.
